Student Driver Application Form

STUDENT DRIVING POLICY
Year 11 and Year 12 students requesting to drive and/or carry passengers to and from Nazareth must complete this Application Form form.

We ask that students and parents/caregivers complete this form together. Once submitted, parents/caregivers will receive an email with a copy of this completed form, for approval.

Permission to drive and carry other passengers is only for the journey to and from school or between campuses during the school day. Students are not permitted to drive, either themselves or other students, to school-related activities during the day that are not held on one of our campuses.

Any student driving another student to and from school or between campuses must have the written permission to do so from the parents of that student.

It is compulsory that the student’s drivers licence, along with the necessary approval to drive and or carry passengers, is received by a Year Level Coordinator before approval to drive to and from school or between campuses is granted.

Please scan a copy of your current Driver’s Licence and email it to student.services_kp@nazareth.catholic.edu.au with your Full Name in the email Subject.

I wish to make an application to drive the above vehicle to and from school or between campuses each day and declare that I am the driver/owner of the above vehicle.

For the privilege of driving, I willingly accept the following conditions:

• I will always park my car in the areas designated as student parking. I am aware that only Year 12 students are allowed to park in the campus carpark and that Year 11 drivers are to park in surrounding areas, ensuring they follow all road rules regarding legal parking requirements and are respectful to neighbours.
• I will not go to my car without permission while at school.
• I will not carry any passengers (ie students) while travelling to or from school or between campuses unless the necessary permission forms have been signed and submitted.
• I will not drive, either myself or other students, to school-related activities during the day that are not held on a Nazareth campus.
• I will drive my vehicle safely and responsibly and will not drive my car in a manner dangerous to the public while in school uniform, remembering that my actions reflect upon the College.

Furthermore, as the driver of this vehicle, I declare that:

• No passenger will occupy a seat not fitted with a seatbelt.
• The vehicle will not be overloaded.
• I am aware of, understand and will abide by the Road Traffic and Safety Rules and Laws regarding driving, drugs and/or alcohol.
• I am aware of the laws regarding the numbers of passengers I am able to transport.
• In the event of an accident, I accept that the College does not accept responsibility for any accident involving this vehicle and myself; furthermore, any passengers are not required to reimburse me for any expenses that may be incurred.
• The Nazareth College Board accepts NO liability for an accident or injuries sustained in an accident that involves private vehicles.
